What is MyMemorialHermann?
Staying healthy begins with staying informed and that means staying connected to your health information - anywhere, anytime. With MyMemorialHermann, it's easy to stay connected to the information that will help you better manage your health.
How do I sign up for MyMemorialHermann?
Whether scheduling an appointment, checking-in, or checking-out, simply provide the registrar or business office with your email address so that we may communicate with you electronically and enroll you in MyMemorialHermann. Typically, the registrar and business office will be located near the entrance/exit of a Memorial Hermann facility or clinic.
Due to sensitive data concerns, 13-18 year old patients are unable to access health information online. If a patient, parent, or legal proxy had requested access prior to the age of 13, MyMemorialHermann will automatically remove access when the patient turns 13. When the patient turns 18, a new MyMemorialHermann account can be requested. A full hard copy of the entire medical record can be accessed by contacting our Release of Information Department. Fees may apply.
What is available on MyMemorialHermann?
Through your browser on a computer, tablet, or smart-phone, you can:
- See a summary of your health record containing allergies, immunizations, medications, health issues, and performed procedures
- View discharge, emergency center, or clinic summary per visit
- View patient education materials
- View lab results
- View radiology reports
- Message participating providers
- Request a medication refill from participating providers
- Find a doctor
- Schedule appointments and follow-ups through ScheduleNow
- Access online bill pay

How do I complete enrollment?
Be sure to check the email account you provided during enrollment. You will receive instructions in an email invitation within 24 hours of enrolling.
- Simply click on the link provided in the email to verify your personal information. The link typically expires within 90 days.
- You will be asked to create a username and password to setup your account. Please store this information in a safe place. The username and password will be required each time you login to the portal.
What if I didn't receive my invitation email?
Please check your spam or junk mail folders in your email account. If you still cannot find it, simply contact us at 1 (877) 621-8014 to verify your email address.
What if I have forgotten my password?
Retrieving your password can be performed online.
- When attempting to login, simply click on "Forgot Password."
- Type in your email address and the words displayed for verification.
- Upon successful verification, you will receive an e-mail containing a link and a few steps to reset the password. The password can be reset within 1 hour of receiving the email.
How do I find my health information?
Once you are signed in, click on "Health Record" to access your health information.
What types of lab results display?
You can access the following most common groups of lab results:
- Urine & Stool
- Tumor Markers
- Body Fluid
While available, the actual results may not have been reviewed by your healthcare professional. Therefore, your lab results alone are not indicative of a medical condition.
Microbiology and pathology reports are not available through MyMemorialHermann.
What types of radiology reports display?
Most radiology reports after January 1, 2015 are available. Recent reports may take up to 4+ days to be displayed.
While available, the actual reports may not have been reviewed by your healthcare professional. Therefore, your reports alone are not indicative of a medical condition.
What is a Visit Care Summary?
A summary of your basic demographics, medications, immunizations, allergies, health issues (known as problems), lab results, procedures and vital signs per each visit with Memorial Hermann that can be downloaded and sent to another provider or third party.
Why can't I see my full historical medical record?
Over time we have been able to use new tools to store patient data in our computer systems known as the electronic medical record. Older records were not stored in a computer system. For this reason, MyMemorial Hermann will have less detail. If you have had a recent visit with us, you will see more detail in your record.
What if I have a clinical question?
Please contact your health provider or hospital at which you had treatment or services performed to verify information and receive clarification. You may visit Find a Doctor to see a listing of all providers and hospitals.
What if my health information is inaccurate?
An amendment form can be completed for changes by visiting our Release of Information FAQ for further instructions.
What if I would like a hard copy of my full record?
A full copy of your entire medical record can be accessed by contacting our Release of Information. Fees may apply.
How is my information protected?
Your health information is protected by State and Federal laws. In addition, MyMemorialHermann uses the latest security technology to keep your health records safe.
Who has access to my online health information?
Only you can have access to your health information. You may grant permission to a caregiver or family member to also view your information. In the future, you will be able to securely transmit your health information to other health providers.
How do I know who has viewed my health information?
When logged into the portal, hover over the settings icon in the top right corner. You will notice an option to "Access Logs." When clicked on, you will see a detailed account of what was accessed, when, and by whom. These logs are made available due to Federal guidelines related to "Meaningful Use."
How much does it cost to obtain access to MyMemorialHermann?
Access to MyMemorialHermann is a free service for patients and health consumers.
How do I deactivate my account?
If you choose to deactivate your online account, simply contact 1 (877) 621-8014.
What is Cerner or CernerHealth?
Cerner is Memorial Hermann's trusted IT partner to deliver your health information online.
What health information can I download?
You may download any files located in the "Documents" section of the portal. In addition, you may download a visit care summary in the "Download" section of the portal. These files typically become available upon discharge at any hospital, emergency, or clinic visit.
